feat(ofrep): set feature flag attributes on the evaluation span - #2106
The OFREP single evaluation handler now sets feature_flag.key, feature_flag.result.variant and the provider name on the request span, using the same SemConvFeatureFlagAttributes helper the gRPC and HTTP evaluators already use.
Only the single evaluation endpoint is changed. The bulk endpoint evaluates many flags in one request, so one set of key and variant attributes does not fit; the gRPC bulk path records a count instead. I can add that here if you want it.

TestHandlerSetsFeatureFlagSpanAttributes records spans for one OFREP evaluation and checks both attributes. It fails without the change.

SemConvFeatureFlagAttributes currently emits feature_flag.provider.name, but TestHandlerSetsFeatureFlagSpanAttributes checks only the key and variant. Removing the provider-name attribute would therefore leave this test passing. The shared helper test also does not require the provider attribute to exist.
